Surrogacy 101: Medical Screening

Surrogacy 101: Medical Screening

Clear. Compassionate. Thorough.

Welcome back to our Surrogacy 101 blog series at Simple Surrogacy, where we walk with you through every stage of the surrogacy journey—from the very first conversation to the unforgettable delivery day and beyond.

Now that you’ve matched with your Intended Parents—whether a loving couple, single parent, or same-sex pair—you’re ready for the next essential phase: Medical Screening.

This is the part of the journey where dreams begin to take root in science and preparation. It may sound clinical at first, but with the right information and support, you’ll find that medical screening is a clear, organized, and empowering step in your surrogacy path. At Simple Surrogacy, we make this phase feel less like a hurdle and more like what it truly is: a moment of affirmation that your body is ready to help create life.

Why Medical Screening Matters

Surrogacy is both a medical and emotional commitment. Medical screening ensures that everyone begins this journey from a place of health, safety, and readiness. For surrogates, this step is designed to confirm:

  • That your reproductive health is in optimal condition
  • That your body can safely carry a child
  • That there are no hidden medical or psychological risks
  • That you’re emotionally prepared for the path ahead

This screening is not a test of perfection—it’s a supportive, informative check-in to ensure that everything is aligned for a successful and smooth journey.

A Supportive Start: What to Expect

The medical screening process begins once you and your matched Intended Parents have officially agreed to move forward. You’ll be scheduled to visit their fertility clinic of choice, where you’ll meet a Reproductive Endocrinologist—a doctor who specializes in helping families grow.

This clinic visit typically involves one or two days of appointments. Depending on how close you live to the clinic, you may travel for the visit—Simple Surrogacy assists with travel arrangements and costs, making this step as convenient and comfortable as possible.

The Gold Standard: Screening by ASRM Guidelines

Your screening will follow the protocols set by the American Society for Reproductive Medicine (ASRM)—the national gold standard in fertility medicine. This means your experience is backed by science, ethics, and safety at every step.

Here’s a breakdown of what’s included:

Step-by-Step: Medical Screening for Surrogates

1. Initial Consultation

You’ll meet with a Reproductive Endocrinologist (RE) who will:

  • Review your health and reproductive history
  • Explain the upcoming steps in detail
  • Answer any medical questions you may have
  • Help you feel confident and informed

This is your time to get comfortable with the medical team and begin building trust. Most surrogates walk away from this visit feeling excited and reassured.

2. Physical and Gynecological Exam

Your RE will perform a full physical exam, including a gynecological check-up. This helps them understand your current health status and ensure there are no concerns that could complicate pregnancy.

What it includes:

  • Pelvic exam
  • Pap smear (if not recently completed)
  • Uterine health evaluation

This part is quick, thorough, and performed by professionals who specialize in working with surrogates every day.

3. Lab Work and Blood Tests

You’ll have several blood draws that provide key insights into your body’s systems and overall wellness. These tests check for:

  • Complete blood count and basic chemistry
  • Blood type and Rh factor
  • Hormone levels (like FSH) during the early part of your menstrual cycle
  • Infectious diseases (HIV, Hepatitis A/B/C, Syphilis, Rubella, CMV)
  • Drug, alcohol, and nicotine screening
  • Immunity status (to ensure you’re protected or can be vaccinated)
  • Genetic carrier testing (Cystic Fibrosis, Tay Sachs, Sickle Cell, and others)

These tests aren’t invasive and can usually be completed in one sitting. The goal is to ensure your safety and the baby’s well-being, and to give doctors a full picture of your health.

4. Ultrasound and Uterine Evaluation

Your uterus will be evaluated using an imaging technique such as a Hysterosalpingogram (HSG) or a Sono-Hysterosalpingogram (Sonohysterogram). These scans help doctors assess:

  • Uterine shape
  • Presence of fibroids or cysts
  • Condition of the uterine lining

Most women report minimal discomfort, and the procedure is typically over in 15–30 minutes. This test gives a detailed look at how well your body can support implantation and pregnancy.

5. Psychological Screening

Surrogacy is a deeply emotional journey, and this screening supports your mental and emotional preparedness. You’ll meet with a psychologist or mental health professional who specializes in third-party reproduction.

What it involves:

  • A conversation about your motivation, expectations, and support system
  • A personality or psychological assessment (often via questionnaire)
  • Guidance on how to navigate the emotional dynamics of surrogacy

This isn’t about “passing” or “failing”—it’s about ensuring you have the tools and support to thrive through the experience.

6. Nurse Coordinator Consultation

You’ll also meet with an IVF nurse coordinator, who will walk you through the next steps of the treatment process. They’ll explain:

  • Your IVF medication protocol
  • How and when to administer hormone injections
  • What to expect during the embryo transfer cycle
  • The full calendar of your journey

You’ll also review and sign the necessary medical consent forms. This part of the visit is incredibly helpful and sets the stage for everything to come.

How Long Does the Process Take?

The medical screening visit itself is usually completed in one or two days. Afterward, lab results typically return within 1–2 weeks. If everything looks good, you’ll be officially medically cleared to move forward with the surrogacy journey.

If anything unusual shows up (like a vitamin deficiency or a previously unknown issue), don’t worry—you’ll have time to address it with your doctor. In most cases, any minor concerns are easily resolved.

What Happens After Medical Clearance?

Once you’re medically cleared, you’ll move on to the next exciting steps:

✅ Psychological Evaluations (if not yet completed)

You and your Intended Parents may participate in joint or individual sessions with a counselor to ensure everyone’s on the same page emotionally.

✅ Legal Contracts

You’ll work with a surrogacy attorney (provided at no cost to you) to draw up clear, protective agreements that outline everyone’s rights, responsibilities, and expectations.

✅ Medication and Embryo Transfer

You’ll begin preparing for the embryo transfer, using a customized protocol of medications (usually estrogen and progesterone). You’ll receive full training and 24/7 support throughout this stage.

Common Questions from Surrogates

Is the medical screening painful?
Most of it is painless! The only mildly uncomfortable part might be the uterine scan, which is over quickly. Everything else feels like a routine doctor’s visit.

Do I have to travel for screening?
If the Intended Parents’ clinic is in another city or state, yes—but we handle all travel, hotels, and logistics. Many surrogates enjoy the brief getaway and use it as “me time.”

What if something is found in my labs?
You’ll be fully informed and supported. In many cases, minor issues (like needing a vaccine booster or vitamin supplement) are easily addressed. If anything major is found, we’ll walk with you compassionately through your options.

Can I bring someone with me?
Absolutely. Many surrogates bring a partner, friend, or family member for support—and we’ll help cover associated travel if needed.
